FCH provides a whole-person health model for patients, which includes medical, optometry, dental and pharmacy services. Additionally, through a partnership with the YMCA, patients get the opportunity to participate in the Healthy Lifestyles Program. This program includes classes which focus on nutrition, exercise, faith, emotions, relationships, and stress management. 
Those interested in becoming clients of FCH are required to attend an orientation session with a Healthy Lifestyles Coach at the Ozark Mountain YMCA in order to register for services through FCH. Eligible clients must provide proof of income status that falls within FCH requirements. For example, a household of 2 with an annual income of $28,000 or less will be eligible. In addition, potential clients must provide proof of employment status within the last 6 months in Stone or Taney County and be uninsured. Client fees will be based on a sliding scale for visits to FCH, including access to prescribed classes at the YMCA.

Will you have a full time physician on staff?
We plan to utilize the services of several practioners in the area and it is our hope we will employ a full time physician and nurse on staff.
Where is the Center located?
Due to a generous donation by Skaggs Regional Medical Center, FCH is located at 610 South Sixth Street near downtown Branson, Missouri. Many people remember this as one of the old Branson elementary schools. For directions to the clinic, please call 417-336-9355 (WELL). The Healthy Lifestyles Program will be held at the Ozark Mountain YMCA, which is located in the Hollister Industrial Park.
How is the Center funded?
FCH is a 501-C 3 organization funded by individual donations, foundation grants, corporate donations and other local businesses. Donations can be mailed to 610 South 6th St, Branson, Mo. 65616. You will receive a tax receipt for your gift.
Is the Center affiliated with a specific denomination?
No, FCH is non-denominational and we will partner with all area congregations to serve both those outside the walls of the church and their congregation members. We will provide pastoral counseling and prayer will be a vital part of our healing and wellness program.
